Your First Appointment
Your first appointment at Belleville Sleep Dentistry will be a consultation with our Registered Nurse. She will review your general health and medical history, go over preoperative instructions and answer any questions you may have. You will then be given paperwork regarding consent for treatment and pre-appointment instructions.
Our office may be in touch with your family physician, to request additional medical information; such as lab reports, in order to ensure that Sleep Dentistry is the best choice for you.
Once all the required medical histories have been received by our office, Dr. Chow will review them carefully to ensure you are eligible for in-office sedation and to make choices tailored specifically to your individual needs. Our office will contact you to inform you whether you have been deemed to be a suitable candidate for in-office general anesthesia.
It is important to note that, with all patients under the age of eighteen and dependent adults, our office requires that a parent or legal guardian accompany the patient for the full duration of the appointment.
A grandparent, childcare provider, another family member or friend cannot authorize treatment on your behalf.

- Our office will not disclose any medical/dental information of a patient—including those patients under 18 years of age—unless it is the parent or legal guardian of the patient.
- A child / dependent adult cannot consent to any required additional procedures, or to any changes in a treatment plan that could arise.
- Our office cannot assume responsibility for the safety, care or whereabouts of a patient once the patient is discharged to a responsible adult caregiver.
